Diamond Dogs Inc. Adoption Contract

I agree to pay the predetermined adoption fee in full at time of adoption. I understand that this fee is not refundable.*

I understand that although Diamond Dogs Inc, henceforth referred to as DDI, exercises exceptional care in evaluating and matching dogs with prospective adopters, DDI cannot guarantee and shall not be held liable for temperament, health, or behavior of above named dog.*

I agree to take exceptional care in providing safe, comfortable living conditions for, as well as to never neglect or abuse the above named dog. I will treat them as an indoor household pet, companion, and family member, and never leave them chained or contained for extended periods of time. *

I agree to never allow the above named dog to wander freely without supervision, to abide by local leash laws, to keep proper identification on them at all times, and to keep microchip registration info updated.*

I agree to seek veterinary care in the event that the above named dog becomes injured or ill and routinely administer preventative modalities in order to protect them from disease and parasites.*

I agree not to abandon, give away, rehome, sell, or in any way dispose of the above named dog, whether to a private party, animal shelter, rescue group, or otherwise. I agree to never euthanize the above named dog except in the case of terminal injury or illness, or pain and suffering due to old age. If at any point it is determined that I can no longer care for the above named dog, I shall relinquish them immediately back to DDI.*

If the above named dog is not spayed or neutered at time of adoption, I agree to have them desexed no later than 36 months of age, and to not allow them to breed under any circumstances at any time. I understand that if I choose to spay or neuter prior to 24 months of age, I may be predisposing the above named dog to joint, ligament, and behavioral complications due to lack of growth hormones and that DDI shall not be held accountable or financially responsible for potential subsequent adverse effects. *

I understand that if I am found to be in breach of any of the terms in this contract DDI reserves the right to recover the above named dog from me on demand. In addition, I will pay DDI a total of $1,000 as just and reasonable liquidated damages.*

Neither DDI, its volunteers, contractors, or officers shall be held liable for any claims, legal actions, losses, damages, or expenses in connection with adoption and ownership of the above named dog.*

This contract is a binding agreement between DDI and myself, as well as my heirs, partners, assigns, successors, personal representatives, and executors. No modification of this contract will be valid unless in writing and duly signed by both a DDI representative and myself. *
